When someone calls for the clinic, staff may collect basic callback information — your name and phone number — so the appointment request can be routed. Clinical questions should be discussed directly with the physician or an appropriate healthcare professional.

Request forms

The refill, transfer, and appointment-request forms collect only what we need to call you back — such as your name, phone number, and the type of request. They are for arranging a callback, not for sending medical details. Please keep symptoms, diagnoses, OHIP numbers, and medication history for the phone conversation. Submissions are delivered to pharmacy or clinic staff by email and are not stored on this website.
